Private Patients’ Medical and Lifestyle Questionnaire
We normally send you this questionnaire before your first appointment with us so we have a full understanding of your hearing loss and any relevant medical history. By completing this form at home you can spend as much time as you need to reflect on the issues which need to be addressed. It is a way of ensuring your audiologist knows everything they need to so they can make the best possible hearing recommedation for you.
Hearbase Hearing Aid Brochure
This is the brochure that we normally give to all our hearing aid patients at their first consultation with us. It tells you about the different types and styles of hearing aids available as well as the pros and cons of various options. You can find out about the different stages of being fitted with hearing aids and learn about our aftercare programme that ensures you get many years of satisfactory hearing aid use.

This brochure tells you about the full range of services which Hearbase offers. We are not just a hearing aid company. We carry out diagnostic tests for hearing and balance problems, provide specialist hearing protection and a full range of industrial hearing services.
One Ashford Hospital Brochure
This brochure is similar in content to our main practice brochure but details our services specifically available at One Ashford Hospital. Retube Your Hearing Aid
If you fancy trying to retube the ear mould of your hearing aid you can find the instructions here. If you need replacement tubing or the tools to do it you can contact our Folkestone office. You can also buy them online here.
